Pharmacy lien FAQ: how it works, costs, and coverage
Frequently asked questions about pharmacy lien services — how liens work, what medications are covered, and how payment is handled at settlement. Answers for attorneys, patients, and healthcare providers.
Attorneys
What is LienScripts?
We're a Pharmacy Benefit Administrator that helps your PI clients get medications at $0 upfront cost, on a lien against their settlement. We handle enrollment, dispensing, refills, and documentation — you focus on the case.
How do I enroll a client?
Simple online form through our attorney portal. Takes about 2 minutes. Once submitted, your client receives an SMS to sign the lien agreement, and we handle everything from there — pharmacy coordination, dispensing, refills, and documentation.
What does it cost?
Attorneys and patients pay nothing upfront. The lien is paid from settlement proceeds. Most firms work with us on a per-patient benefit limit rather than approving each medication individually.
How LienScripts works for firms
What if the case doesn't settle?
We assume that risk. It's built into our business model. We understand that not every case wins, and we don't pursue patients personally for uncollected liens.
What's a MERIT report?
MERIT (Medication Evaluation & Rationale for Injury Treatment) is a clinical narrative that explains your client's medication history — why they needed it, how long treatment continued, and what the patterns indicate about their condition. It's pharmacist-authored documentation designed to support case value, not just an invoice.
Can my clients use any pharmacy?
Yes — 70,000+ retail pharmacies nationwide including CVS, Walgreens, Rite Aid, Costco, and many more. We also offer mail-order delivery for most medications. Note: Controlled/scheduled medications must be picked up at a retail pharmacy (federal law prohibits mailing them).
What states do you cover?
All 50 states. We're based in California but serve clients nationwide through our network pharmacies and mail-order options.

How do you determine pricing?
Pricing varies by medication, pharmacy, and case complexity. We're not insurance — we front the capital and carry the risk, which is built into our pricing. Contact us to discuss arrangements that work for your practice.
How long does enrollment take?
Patients typically sign the lien within 24-48 hours via SMS. The first dispense can happen shortly after, depending on prescription availability and pharmacy logistics.
Do you handle refills?
Yes — we manage ongoing refills for the entire duration of the case. Patients don't need to coordinate anything; we track their prescription schedules and ensure continuity of care.
What about Medi-Cal/Medicare liability?
By using LienScripts instead of government insurance for PI-related medications, you avoid third-party liability reporting and government lien complications at settlement. One lienholder (us) instead of multiple. Clean settlement math.
Patients
Do I have to pay anything now?
No. $0 upfront. Your attorney has enrolled you in LienScripts, which means your medications are provided through a medication agreement — a promise to pay from your settlement when your case resolves.
When do I pay?
Only when your case settles. The medication costs are paid from your settlement proceeds, and your attorney handles this as part of the settlement process. You never pay out of pocket.
How do I get my medications?
Two options: Mail delivery to your home (for most medications), or pickup at any pharmacy in our network — that's 70,000+ locations nationwide including CVS, Walgreens, Rite Aid, and Costco.
How your digital pharmacy card works
Is this legit?
Absolutely. LienScripts works with licensed pharmacies nationwide to help personal injury patients get their medications. Your attorney set this up for you as part of your case. We work with law firms across the country.
What if I need a refill?
We handle refills automatically based on your prescription schedule. If you need something sooner or have a new prescription, contact us and we'll coordinate with your doctor and pharmacy.
What if my case doesn't settle?
You won't owe us anything. We take on that risk as part of how we operate. If your case doesn't result in a settlement or recovery, we don't pursue you for payment.
Can I get controlled medications (pain medications, etc.)?
Yes, but controlled/scheduled medications must be picked up at a retail pharmacy. Federal law prohibits mailing these medications, so you'll pick them up at a convenient network pharmacy near you.
What if I have questions about my medications?
Contact us anytime. We have pharmacists on staff who can answer questions about your prescriptions, potential interactions, or how to take your medications properly.
Can I use my regular pharmacy?
If your regular pharmacy is in our network (most major chains are), yes. We have 70,000+ locations nationwide. If not, we'll find a convenient alternative near you.
Providers
How do I refer patients to LienScripts?
Contact us to set up a partnership. We'll provide referral materials, explain the process, and give you access to our provider portal to track patient status.
How the referral and dispensing process works
How do I prescribe for LienScripts patients?
Prescribe normally through your existing workflow. E-prescribe, fax, or call in — we accept all methods. We'll coordinate with your preferred pharmacy or our network pharmacies for dispensing.
Does this cost my practice anything?
No. There's no cost to the provider. You prescribe, we handle the lien coordination and dispensing, and the patient gets their medications without paying out of pocket.
Can I track my patients' medication status?
Yes — provider portal access is available for partner clinics. You can see what's been dispensed, when, and confirm your patients are getting their medications.
How does this help my patients?
PI patients often skip or delay medications when they can't afford them, which hurts outcomes and creates follow-up burden for you. LienScripts removes the cost barrier so patients actually take the medications you prescribe.
What medications are covered?
Most medications related to personal injury treatment: pain medications, muscle relaxants, anti-inflammatories, nerve pain medications, topical treatments, and more. Contact us if you're unsure about a specific medication.
